Authorize the Department of Health Services (DHS) to accept the California Department of Public Health, Maternal, Child and Adolescent Health Division (CDPH/MCAH) grant award for $375,000 and First Five Yolo grant award for $50,000 to fund the California Home Visiting Program; authorize the Department of Health Services Director and/or her/his designee to sign any and all documents necessary to accept the grant awards; approve and authorize the Chair to sign Agreement with Yolo County Children’s Alliance in the amount of $385,000 to provide California Home Visiting Program Services for the period July 1, 2015 through June 30, 2016; and authorize the Department of Health Services Director and/or her/his designee to sign any additional documents necessary to execute the Agreement and any amendments to the Yolo County Children’s Alliance Agreement.
